We are proud to accept Colorado Medicaid and Private Pay for our ILST Services.
Independent Living Skills Training (ILST) is a Colorado Medicaid program specifically designed for those who have suffered a traumatic or acquired brain injury to learn the skills necessary to live safely and independently again. Our ILST program helps bridge the gap between families and caregivers with care that empowers, encourages, and engages our clients.
ILST can be a life-changing program for people recovering from acquired or traumatic brain injuries.
Our program is designed to provide support services to help our clients rebuild and develop the skills necessary to live independently.
We understand that every individual’s needs are unique, which is why we work closely with our clients and their families to develop personalized Plans of Care that are tailored to meet their own needs and goals.
What is ILST?
Andrea’s Angels works with Colorado’s ILST (Independent Living Skills Training) program to help our clients develop independence while meeting each individual’s unique needs.
We create a customized Plan of Care based on each client’s needs. Each plan may include support in:
- Self-care, including but not limited to basic personal hygiene
- Medication supervision and reminders
- Household management (paying bills, setting up appointments, maintenance needs)
- Time management skills training
- Safety awareness skill development and training
- Task completion skill development and training
- Communication skill building; (starting and ending conversations, staying on topic, eye contact, turn-taking)
- Socialization, including but not limited to acquiring and developing appropriate social norms, values, and skills
- Recreation, including leisure and community integration activities
- Sensory motor skill development
- Benefits coordination, including activities related to the coordination of Medicaid services
- Resource coordination, including activities related to coordination of community transportation, community meetings, neighborhood resources, and other available public and private resources
- Self Advocacy
- Observing Infection Control Practices
The ILST Process
- In-Home Engagement Assessment
- Each ILST client receives an Engagement Assessment so the whole care team, including the family, can learn how to better support that individual
- Side-by-side care plan development
- We involve the patient’s whole team, including family, in the planning process
- Comprehensive ILST Assessments
- These take place throughout all stages of care
- Resources & service recommendations
- From our memory care experts
- Care Coordination with your Care Team of Professionals such as:
- Occupational Therapist
- Physical Therapist
- Registered Nurse
- Speech Language Pathologist
- Psychologist
- Neuropsychologist
- Medical Doctor
- Licensed Clinical Social Worker
- Licensed Professional Counselor
- Quarterly visits with our ILST Care Team
What are the eligibility requirements to enroll in ILST?
To be eligible for this service, you must:
- Be an adult between the ages of 18 and 65; and
- Have suffered from a brain injury that has resulted in permanent disability
- Be eligible for Medicaid
- Not be able to perform three or more activities of daily living (ADLs) without assistance or supervision
- Demonstrate issues with thinking or learning that interfere with independent living skills
- Demonstrate issues with communication that interfere with independent living skills
