Transition Coordination Services (TCS) assist people who want to return to the community from a nursing home setting. It helps people transition out of nursing homes and avoid such placement. TCS is a benefit of the Health First Colorado (Colorado Medicaid) program.
According to the AARP, 77 percent of Americans want to stay in their homes as they age. At the same time, the number of residents in nursing homes has decreased by 12% in the past eight years, signaling a growing preference for home living among seniors. What Is TCS?
TCS is a Colorado Medicaid-funded program designed to help people move from a long-term care facility like a nursing home to live in their homes and communities. The program is offered to Medicaid members living in nursing homes.

Who Qualifies for TCS?
TCS is available to Coloradans who meet specific criteria. To qualify for TCS, individuals need to:
· Have Medicaid
· Be willing to move from an institutional setting (e.g., a nursing home) to a community-based dwelling (e.g., their own home).
· Have health needs that require assistance to live independently
· Be eligible for Colorado’s Home and Community Based Services (HCBS) Waivers, including: Elderly, Blind and Disabled (EBD) Waiver, Brain Injury Waiver, or Community Mental Health Waiver
· Agree to work alongside a TCS worker to develop a transition plan.
