Transition Coordination Services with Andrea's Angels
We accept Colorado Medicaid and Private Pay for our TCS Services
Transition Coordination Services (TCS) is a Medicaid-funded program designed to help people who are recovering from an acquired or traumatic brain injury move from a long-term care facility back into the community.
Andrea’s Angels’ TCS program is designed to be flexible and individualized, and we work with you to determine what kind of assistance you need as you or your loved one transition back into the home. TCS provides support, information, and guidance to help you understand what you need to do in order to live independently in your own home.
Transition Coordination Services
The Transition Coordination Services (TCS) program helps people transition back into their own homes after recovering from acquired or traumatic brain injuries. It provides assistance with:
- Assessment: Accurately evaluating a client’s strengths, needs, preferences, supports, and desired outcomes.
- Service Planning: Developing a specific Plan of Care that enables clients to meet their goals
- Referral: Informing clients of their options so that they can choose a qualified provider and coordination with that provider to ensure the Plan of Care is followed
- Monitoring and Remediation: Ensuring clients receive services they are entitled to and that these services meet their needs and goals. We work to anticipate potential barriers to care, and work to address problems as they arise
- Other Transition Coordination: We advocate for clients throughout their care journey
What are the benefits of TCS?
The Transition Coordination Services (TCS) program can help eligible clients:
- Move from a hospital, nursing home, or other long-term care facility to make their way back into their homes and communities
- Empower and engage clients as they navigate the transition from facility to home
- Serve as advocates on behalf of clients while they navigate the healthcare system
What are the eligibility requirements to enroll in TCS?
Clients must meet the following criteria to be eligible for Transition Coordination Services:
- Have expressed interest in moving to a community-based setting
- Be 18 years of age or older
- Determined financially eligible for Health First Colorado (Medicaid-eligible)
We are equipped and happy to help long-term care facility residents who receive Medicaid benefits transition from facility into the community. Our dedicated team of “Angels” are trained and highly skilled at helping people regain a sense of independence and community after a health emergency or hospitalization.
