Transition Coordination Services (TCS) is a Medicaid-funded program designed to help people who are recovering from an acquired or traumatic brain injury move from a long-term care facility back into the community.
Andrea’s Angels’ TCS program is designed to be flexible and individualized, and we work with you to determine what kind of assistance you need as you or your loved one transition back into the home. TCS provides support, information, and guidance to help you understand what you need to do in order to live independently in your own home.
